  • Group policies must provide continuation coverage to an employee and his or her spouse and dependents for 12 months if the employee is on leave without pay due to a total disability. See Disability Leave.
  • Group health policies must include a provision that allows employees and their spouses and dependent children to convert from group health coverage to individual coverage. See Conversion Rights.
