No. Employers may not ask prospective employees to fill out medical questionnaires. Forms or questionnaires given to applicants before a job offer is extended that ask questions about physical and/or mental conditions, past or present, are considered an invasion of privacy and violate the Americans with Disabilities Act (ADA).
