Early Termination of COBRA Coverage Letter

When to Use This Letter
This letter notifies a participant and/or qualified beneficiary on COBRA continuation coverage that his or her COBRA coverage will terminate earlier than the end of the maximum coverage period of COBRA coverage applicable to a qualifying event. The notice should be sent to the covered individual(s) as soon as practicable following the plan administrator's determination that continuation coverage will terminate. The notice must be written in a manner calculated to be understood by the average plan participant and must explain the reason continuation coverage has terminated; provide the date of termination of continuation coverage; and describe any rights the qualified beneficiary may have to elect alternative group or individual coverage, such as a conversion right.
