Fair Credit Reporting Act Summary of Rights Form
Entity Mandating Form: Consumer Financial Protection Bureau (CFPB)
Applicability: Before taking an adverse action against an applicant or employee based on a consumer credit report obtained from a consumer reporting agency, an employer must first provide the individual with, among other things, a summary of the individual's rights under the Fair Credit Reporting Act. An employer must also provide the summary of rights within three days of requesting an investigative consumer report from a consumer reporting agency.
