New York City Earned Sick Time Act Employee Request Form to Make Up Missed Work as Alternative to Using Sick Leave

Applicability: Under New York City's Earned Sick Time Act, an employer may require an employee to sign a form if the employee wants to work additional hours instead of using earned sick leave. An employer may not require an employee to work additional hours or make up missed hours; both the employer and employee must agree to it.
