New York City Fair Chance Model Notice
Entity Mandating Notice: New York City Commission on Human Rights.
Applicability: An employer with four or more employees in New York City is covered by the notice provisions of the Fair Chance Act.
Distribution Specifications: If an employer is seeking to withdraw a conditional employment offer due to an applicant's criminal records based on its evaluation in accordance with New York Correction Law Article 23-A, prior to withdrawing the offer an employer must provide notice of its analysis of the applicant's criminal history. An employer may adapt the notice to its preferred format so long as the substance of the notice is not changed.
