When to Use
The ability to work from home or other remote location can provide benefits for both an employee and the employer. For an employee, it may reduce commuting time or provide greater flexibility. For an employer, it may result in increased productivity and reduced absenteeism. In addition, providing this benefit may increase the perception of an employer as an "employer of choice."
This form may be used to allow an employee to request a work-from-home arrangement. It provides a means for the employer to gather the information needed to make a determination on whether to deny the request or to move forward with a formal telecommuting agreement and to document the decision.
