Background Check Disclosure and Authorization Form With Parental Consent

When to Use

When conducting a background check on a minor, an employer should use this disclosure and authorization form in order to comply with the federal Fair Credit Reporting Act (FCRA). The FCRA has strict language requirements regarding disclosure and authorization forms.

An employer should use this form prior to requesting a background check from a third-party consumer reporting agency on a minor aged under 18. In order to conduct a FCRA-compliant background check on a minor aged under 18, an employer will need to have the minor's parent or legal guardian complete and sign an authorization form.

Employers may not request a background check from a consumer reporting agency without first providing the proper disclosure and obtaining authorization from the parent or guardian of the subject of the report.
