When to Use
The Periodic Acknowledgment form should be used uniformly across the entire employee pool, at regular, repeating intervals. The recommended usage for this form is once per year in connection with annual performance evaluations, performance or business practices appraisals or other organized employer activities designed to obtain feedback from or provide feedback to employees.
If used properly and effectively, this form can function much like an exit interview conducted at the end of a regular interval of employment.
New employees should also execute this form as soon as possible after they are formally brought into the organization. They can then reevaluate and execute the document at the next designated interval.
