Possible Employee Exposure to COVID-19 Letter

When to Use
Employers should use this letter to notify employees that an employee in the organization or at a particular worksite location tested positive for the novel coronavirus (COVID-19) and that they had close contact with this employee. Close contact is defined by the Centers for Disease Control and Prevention (CDC) as being within six feet of someone who has COVID-19 for a total of 15 minutes or more.
