  • Alaska Separation Notice to Employees

    As mandated by the Alaska Department of Labor and Workforce Development's Division of Employment and Training Services, all employers are required to provide this Alaska Separation Notice to Employees.

  • Qualified Small Employer HRA Model Notice

    Under the 21st Century Cures Act, a small employer that has fewer than 50 full-time employees (including full-time equivalent employees) and that does not offer group health insurance to its active employees may provide stand-alone qualified small employer health reimbursement arrangements (QSEHRAs). An employer may use this model notice to fulfill the annual notice requirement.

  • Medicare Part D Creditable Coverage Notice

    Entities that provide prescription drug coverage to Medicare Part D-eligible individuals must disclose to those individuals whether their drug coverage is creditable or noncreditable prior to October 15 each year, when an individual joins the plan and upon the individual's request.

  • Connecticut Unemployment Separation Package (Notice UC-61)

    As mandated by the Connecticut Department of Labor, all employers should provide the Connecticut Unemployment Separation Package (Notice UC-61) to employees upon termination.

  • Georgia Mass Separation Notice (DOL 402)

    An employer must file this form with the Georgia Department of Labor within 48 hours following a mass separation of 25 or more employees in one establishment on the same day for the same reason (other than a labor dispute).

  • Georgia Mass Separation Notice - List of Workers (DOL 402A)

    An employer must file this form with the Georgia Department of Labor, in conjunction with DOL 402, within 48 hours following a mass separation of 25 or more employees in one establishment on the same day for the same reason (other than a labor dispute).

  • Georgia Individual Separation Notice (DOL 800)

    Outside of mass terminations, an employer must provide all separated employees with this form on the employee's last day of work or must mail it to the employee within three days.

  • Notice for Employer-Sponsored Wellness Programs

    As mandated by the Equal Employment Opportunity Commission, an employer that offers a wellness program that collects employee health information must provide a Notice for Employer-Sponsored Wellness Programs.

  • California Notice of Election to Make Voluntary Plan Applicable to All Employees, DE 2520CV

    An employer with a certain percentage of employees who have consented to a voluntary plan for payment of disability insurance and family leave benefits, must provide written notice to new hires.

  • COBRA General Notice

    Employees and their spouses must receive a general notice of rights under the Consolidated Omnibus Budget Reconciliation Act (COBRA) within 90 days of the beginning of group health plan coverage.
