When to Use This Letter and Form

Employers may use this letter to notify employees that direct deposit is available. The accompanying form may be used to obtain employee authorization to be enrolled in direct deposit, to add a new direct deposit account, or to change or delete an existing direct deposit account.

While many employees enjoy the ease and convenience of being paid by direct deposit, some employees resist it. Letters like this one, sent at regular intervals can help convince resistant employees to enroll in the employer's program.
