Oregon Employment Insurance Notice (Form 11) Poster
Entity Mandating Poster: The State of Oregon Employment Department.
Applicability: In general, employers with (i) at least a $1000 payroll in a calendar quarter or (ii) employers with one or more employees in each of 18 weeks in a calendar year. However, there are different requirements for agricultural and domestic employers.
Where to Post: The poster must be displayed where employees can easily see it.
Obtaining This Poster: The form should be provided to an employer once the department determines the employer to be covered under the law. In addition, an employer may order the poster on the State of Oregon Employment Department website or by contacting the Department at 503-947-1488, Option 3. When ordering, employers should have their business identification number ready when they call.
